The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

New Patient Registration at Shaftesbury Medical Centre

registrationPatients who live within our practice catchment area will be asked to produce their medical card or NHS number if possible. If you do not have a medical card we will need details of your previous address and previous doctor as well as your NHS number.

If you have never been registered with a doctor in this country before you will be asked to produce your passport and/or immigration papers. You may also be asked to provide proof of residency e.g. utility bill.

Patients will be registered with the Shaftesbury Medical Centre and may consult with any available doctor. Patients have the right to express a preferred GP that they wish to see and manage their care. Patients may have to wait longer to see their preferred GP.

At registration you will be given a health questionnaire to complete and will be offered an appointment for a health check up.

To save time you may print the registration form by following the link below and then fill it out at home. However, please note that you must bring the form into the surgery in person with proof of your identity and address.
