Routine appointments
To request a routine appointment in the next 7 days
- use our appointment request form, Monday to Friday from 8am to 6:30pm. We will respond within 2 working days during opening times

Your appointment
However you choose to contact us we may offer you a consultation:
- by phone
- face to face at the surgery
- by text.
Appointments by phone or by text l can be more flexible and often means the doctor or nurse can help you sooner.
To cancel your appointment:
- use your Patient Access account (on the Patient Access website or in the Patient Access app.)
- fill in an online form.
We offer extended access appointments on Saturday morning from 9am to midday.
We are also open once every 6 to 8 weeks on Saturday afternoon from 1pm to 5pm.
Harrow Collaborative PCN offer evening and weekend appointments through their Extended Hours Hub service. Patients who are registered with Shaftesbury Medical Centre have access to evening and weekend GP appointments.
We are open daily for out of hours appointments (6:30pm to 8pm), and Saturday (9am to 5pm). If you would like to have an appointment during these hours, please contact your GP surgery for the first available appointment.
Depending on your need you might see a doctor, nurse, health care assistant, clinical pharmacist, physician associate, counsellor or a physio.
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
- If there’s a specific doctor, nurse or other health professional you would prefer to respond.
- If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need. To request a home visit, it’s helpful if you phone the practice before 11am on 0208 423 5500.
